Support Worker
Up to £19,888 per annum
35 hours per week (typically Monday to Friday 9am – 5pm)
Fixed term until 31st March 2024

Your new role
You will be supporting patients who have been discharged from hospital and reducing the risk of needing another hospital visit. This support may include collecting prescriptions, making sure they have food in the house, light household tasks and making sure they know who to contact should they need more help.

  • Speaking with healthcare professionals about the support patients require.
  • Driving to different locations including hospital, doctor surgeries, pharmacies, shops and patient's homes.
  • Offering both emotional and practical support to people using our service. (This role does not involve personal care. You will not be responsible for bathing or cleaning clients)
  • Ensuring records are kept updated with service users care.
  • Helping to assess service users future needs and helping develop support plans.

What you'll need to succeed

  • Experience in a similar role
  • A driving licence as well as your own transport.
  • Be caring and empathetic to be able to offer the best support.
  • Good IT skills
  • An understanding of the services the NHS provide, to be able to offer the best advice and guidance to our service users.
  • A flexible approach: must be able to cover shift patterns between 9am - 5pm and 12pm - 7pm, Monday to Friday.
